Expect delivery by mail within 6-10 business days.

Label the collection card with your name and the date.

Fill out the Requisition Form; under Payment, choose “Practitioner” for Payment to Quicksilver.

Allow the sample card to air dry for at least 30 minutes before placing it in the prepaid shipping envelope.

Deliver the sample to your nearest FedEx Express collection site within 24 hours of preparation.

Once Quicksilver Scientific receives your sample, you will receive an email with a username and a link to create a password for their Patient Portal, where you can track your test status.
